This resource helps allogeneic and autologous transplant recipients understand and prepare for their post-transplant follow-up care. The toolkit contains two easy-to-understand guides that provide the recommended tests and evaluations for six-month and annual (12+ month) appointments—available in multiple formats (print, online, and mobile app*). These guidelines are automatically mailed to unrelated transplant patients who received a transplant through NMDP/Be The Match.
